Athletic Insurance Policy

Dear Student- Athlete and Family, 

This letter is to provide you with the insurance policy for the 2026-2027 school year. Similar to last year, insurance will be verified by the athletic trainers beginning June 1. All summer paperwork is due July 15 (more information will be sent out when the spring semester ends). Please review the insurance information below. All insurance must be verified each year “even if your insurance did not change”. If you have any questions about the policy please reach out to the athletic trainers. 

  • Covered In-network: primary care, routine care, orthopedics, x-rays, advanced imaging, surgery, inpatient and outpatient services, emergency and NON-emergency services (routine services, including wellness and preventative etc.) in the Upstate Region (Spartanburg or Greenville Counties) as a primary health insurance plan when a secondary policy is present 
  • The in-network coverage should have the Converse University team physicians (Prisma Health), local plans which have other Upstate Region physicians could be accepted, but athletes take the risk of delay in treatment and return to sport. If there are no upstate region (Spartanburg or Greenville Counties) physicians in-network, the plan cannot be accepted 
  • Must have coverage for intercollegiate sport/athletics injury coverages 
    • If there are limitations to intercollegiate sport injury coverages, a supplemental plan must be purchased in addition to having primary health insurance that has intercollegiate sport coverage 
  •  Cannot have any waiting periods of pre-existing conditions 
  • Cannot be a reimbursement plan (must explicitly explain direct payors in the USA) 
  • Cannot be a health share plan 
  • Cannot be a non-local HMO without Away from Home (or Guest) Policy 
  • Cannot be a government funded or sponsored health plan (Medicaid (in-state and out-of-state), Medicare, Tricare); Due to a federal mandate that requires a priority reversal -Medicaid and Tricare become the secondary coverage when someone has any other insurance policy in conjunction with the above-mentioned insurances. 

Secondary Insurance Policy Procedure

All student-athletes who have been cleared to participate and meet the above insurance requirements will  also be covered by Converse University’s Secondary Excess Insurance policy in the event of an athletic injury.

  1. The student-athlete will only be covered by Converse’s secondary excess insurance policy if the injury is athletic related and qualifies under the terms of Converse University’s then-current secondary insurance policy which is incorporated herein by reference. A claim form will be filled out by the Sports Medicine Department.
  2. Injuries are considered athletic-related when the injury occurs during official practice,
  3. conditioning sessions, or games. 
  4. This policy will NOT cover the following: Intramural sports or activities, recreational
  5. activities, conditioning/training outside the competitive season not supervised by
  6. Converse’s Strength and Conditioning Coach, and the NCAA off-season period.
  7. The policy does NOT cover any pre-existing condition and/or injuries found during the
  8. pre-participation physical performed by Converse physicians.
  9. This policy does NOT cover illnesses.
  10. If a student-athlete suffers an athletic-injury, they MUST be evaluated first by the Converse University Sports Medicine Department. Upon evaluation, a staff member will make the referral, if needed, to a Team Physician.
  11. If the student-athlete chooses to see a doctor outside the Converse University physician staff without authorization from the Sports Medicine Department, those bills will be the student-athlete’s responsibility.
  12. A Converse University Team Physician must refer a student-athlete to another physician for the secondary excess insurance to cover the remaining charges.
  13. If a student-athlete wants a second opinion from another physician (without being referred), this will NOT be covered by Converse’s secondary excess insurance.
  14. Any injury that the athletic trainer is not notified of before leaving for the summer will not be covered by Converse’s secondary policy.
  15. Athletic injuries covered under our secondary insurance policy will receive 52 weeks of coverage from the date of injury.

If your primary health insurance does not meet these requirements, please reach out to the athletic trainers. They will help to assist you to achieve compliance with your specific health plan. 

For your insurance to be approved we need 2 items: 

  1. Clear images of the front and back of your current insurance card 
  2. Proof of Active Insurance which matches the insurance card 
  3. Proof of Active Insurance (PAI): could be presented in 3 ways: 
  • A dated letter from your insurance carrier (not more than 30 days old) stating you are actively covered under this insurance policy. Must include the policy name and member ID number 
  • A dated letter from your parents’ employer (not more than 30 days old) stating you are actively covered under this insurance policy. Must include the policy name and member ID number 
  • A screen shot from a laptop/desktop (NOT A MOBILE PHONE) of your benefits/insurance website showing you are actively covered with the date the image was made (within 30 days) & the effective date of your coverage, clearly shown in the image, must include the policy name and member ID number or name. 

This letter is for informative purposes only. No action is needed until summer paperwork information and directions are sent out.
